Monterey Bay Geriatric
Resource Center, Inc., (MBayGRC), a non-profit regional
consortium of health care and educational organizations in the
Monterey Bay of California, was formed in 2002 with a grant from
Central Coast Area Health Education Center (CCAHEC). MBayGRC is
dedicated to the advancement of geriatric and chronic care
management programs and services.
MBayGRC's two-fold approach is to:

develop interdisciplinary geriatric/chronic care management
educational programs for family practice residents, nurses,
allied health providers, seniors, family and informal
caregivers, and |

2) |
develop geriatric/chronic care
management centers to provide comprehensive assessments and
chronic care management services to seniors, their families
and providers that enhance or augment a community based
continuum of care. |

Our Mission:
MBayGRC's mission is to create important, new and sustainable educational
and geriatric/chronic care management resources to improve access and care
for an under-served and rapidly growing multicultural, high-risk senior
population in Monterey, Santa Cruz and San Benito counties. |
