Shaping the Future of Geriatric and Chronic Care Services

What We Do

MBayGRC's current Programs and Services include:

  • Developing and beta testing of geriatric core curriculum for the 3 year UCSF family practice residency program at Natividad Medical Center.

  • Developing and beta testing of geriatric core curriculum for the 2 year associate degree in nursing program at Cabrillo College.

  • Developing a Geriatric Assessment and Care Management Center at Community Hospital of the Monterey Peninsula's Behavioral Outpatient facility for older adults with mood disorders and multiple co-morbidities.

  • Developing a feasibility study and business plan for a Geriatric Evaluation and Chronic Care Management Inpatient Unit at Natividad Medical Center.

  • Sponsoring Annual Conferences on Aging and Chronic Care Management for physicians, nurses, behavioral and allied health professionals.

  • Sponsoring workshops/seminars in geriatrics and chronic care management for health care staff and community service agencies that provide support services to frail elderly and family caregivers.

  • Developing a Geriatric Assessment/Chronic Care Management Center at the VA Clinic in Monterey to provide patient services and to serve as a training site for family practice residents and nursing students.

  • Sponsoring geriatric fellowships for graduates of UCSF's family practice residency program at Natividad Medical Center to become MBGRC faculty.

MBayGRC staff who are experts in geriatrics and chronic care management provide strategic planning, program development, training and technical assistance to MBayGRC corporate partners and affiliated health care and professional educational organizations in the tri-county area.
